Each thread should be for an individual character. In that thread, each post (or you can edit to add extra entries if nobody has posted after you) should contain the following information:
1. Offense
2. Punishment enacted
3. Date performed
Name is not required as it should be in the thread title itself.
If you add a person who isn't listed in the index thread, update that thread as well to include the new name/player.
